Forms to be completed and turned in by or before September 18th, 2017:

Forms Cover Sheet Checklist(This form is to help keep track of the rest of the forms that need to be turned in, but does not itself need to be turned in.)

FCPS Emergency Care Form (A separate one must be kept on file in the choir office for all related curricular, co-curricular, and extra-curricular activities.)

If your child already has an emergency care form on file with FCPS, you can print the completed form by doing the following:

 – Login to FCPS 24/7 Learning.

 – On the right -hand side, click “Enter wecare@school”

 – To the right of your child’s name, click View The Form

 – Print the form

 – Instructions for updating emergency care can also be found by following steps 1 & 2

FCPS Extra Curricular Participation Form (Required by FCPS.)

FCPS Parent Authorization Field Trip Form (Required by FCPS. This one form will provide authorization for all SLHS Chorus events & performances for the entire year.)

FCPS Driver/Vehicle Authorization – Form A for Adult Driver (required by FCPS for all students & parents who may carpool students throughout the year for school related events and all student drivers.)

FCPS Driver/Vehicle Authorization – Forms for Age 16/17 Driver) (required by FCPS for all student drivers)

Electronic Student Information and Policies Acknowledgement and Release Form (Required by Parents for Choral Arts, following FCPS protocol

Returning Student Uniform Order Form (All returning students are strongly encouraged to be sized in case they need to order replacement uniform pieces. Order deadline is Friday, September 9th.)

Electronic T-Shirt Form (All students receive a t-shirt.)
